During COVID-19 We are providing Virtual Visits to New Patients. In order for your Virtual Visit to start you will need to complete the following forms and submit this to our office along with a copy of your INSURANCE CARD and PHOTO ID 1 hour prior to your appointment. If we don’t have your completed registration before your appointment time we will need to reschedule your appointment.

  1. Log into your Patient Portal and complete “Personal Information, Additional Information, Medical History, and Surgical History”
  2. After you have completed your information on the Patient Portal you will need to submit a copy (or photo) of your insurance card and photo ID along with  the Registration Form and Medication List To submit a copy of your cards and forms you can e-mail them to cardinalinternalmedicine@gmail.com or fax them to 703.897.6603
  3. After you have submitted everything our office will call you 15 to 30 mins prior to your appointment to collect your co-pay you will then need to start your Virtual Visit.
  4. You will be asked to enter your vital signs so if you are able to check your weight, height, temperature, blood pressure, or heart rate at home please do so before you start the visit.
